Important information
- This form must be completed individually for all travellers aged 1 year and older who are registered at the practice.
- Please submit your travel risk assessment at least 12 weeks before your departure date.
- If you are travelling in under 12 weeks, we may not be able to accommodate your request. You can request your vaccination history from us and arrange your vaccinations at a private travel clinic. Charges may apply.
Before you start
We’ll ask you for:
- your first and last name, date of birth, sex, postcode, email and phone number
- if applicable, the details of the person you are completing the form on behalf of
- information about your travel plans, including destinations and length of stay
- your medical history, including any conditions, medications, or previous vaccinations
