Planning a trip abroad? It is important to arrange a travel risk assessment at least 12 weeks before your departure date. This ensures you receive the necessary vaccinations, health advice, and preventive treatments specific to your destination.
NHS-funded travel vaccines
We only provide the following NHS funded travel vaccinations:
- Hepatitis A
- Typhoid
- Tetanus, Polio & Diphtheria
- Cholera
To receive these vaccines, you must complete and submit a travel risk assessment at least 12 weeks before your scheduled travel date.
Steps to follow
Complete the travel risk assessment form
All travellers over the age of 1 must complete a travel risk assessment individually. This form must be filled out by both the patient and the private travel clinic provider.
How to obtain the form
- Fill out the travel risk assessment form online.
- Download and print the travel risk assessment form and complete it manually.
- Collect a copy from our reception desk at either surgery.
Review & appointment
- The practice nurse will review your form.
- If necessary, an appointment will be arranged for NHS-funded vaccines only.
Travelling in under 12 weeks?
If you are travelling in under 12 weeks, we may not be able to accommodate your request.
You can:
- Request your full vaccination history from us.
- Take this information to a private travel clinic for all necessary vaccinations.

We are not a travel clinic and do not provide private travel vaccinations. Please visit a private travel clinic for a full risk assessment and additional vaccinations as required.
