If you are travelling abroad and require vaccinations, please complete the travel vaccination form no later than 12 weeks prior to your departure date. This is due to the ordering of vaccines on an individual requirement basis. Any vaccines required under 12 weeks of departure date, patients will be asked to seek advice and book a consultation with a local travel centre.
